This form will help you to formally request any information that the state or individual authorities are obliged to disclose under the wording of the Act. The obligation to disclose does not apply to classified information, trade secrets, etc., see detailed restrictions in the law. However, the authority in question must in any case respond to your request within two weeks and, in the event of failure to provide the information, justify its refusal. You will receive the reply in your mailbox.

What you need

  • Being a natural person, not a legal entity (corporation, foundation, etc.)
  • Citizen identity for logging into the Citizen Portal
  • Databox connected to the Citizen Portal

How to do it

On the homepage of the Citizen's Portal, type "Submit a request for information" into the search engine.

You will see a form containing your identification data, below you will find the "Request Detail", fill in the subject of the message and the text of your request. You will then enter the addressee, which is the name of the public authority, whose full name will automatically appear in the directory, including the databox identifier.

You can send your request by data message or download it from the Citizen's Portal and deliver it by post.
